Ⅴ. Note:

1. Order of arrangement and binding of application materials. All application materials shall be arranged and numbered according to the order of materials listed above. Enterprises making declaration for the first time can bind application materials after telephone counseling. Every page of the application materials should be stamped with the official seal of the enterprise. For overly long contracts, it can be stamped with the seal of perforation instead.

2. Original copies of materials. Original copies of materials are required to be submitted when enterprises send declaration materials to the Shanghai Municipal Commission of Commerce. The original copies will be returned to enterprises after examination. (For materials such as customs declaration that have been handed over to relevant departments or the original copy has not been obtained from customs, enterprises should borrow or get the original copy by negotiating with relevant departments.)

3. Materials submitted. Enterprises should prepare two copies of written application materials and one copy of electronic data of Attachment 2, 3, and 4, and send all materials to the Shanghai Municipal Commission of Commerce by the end of Aug 5. (Please check the numbers carefully. Responsibility falls on applying enterprises for any results caused due to incorrect information.)  

4. Requirements for electronic data. The declaration enterprise shall fill in the form attached to the notice and shall not change the format or any content on the forms. If the system fails to recognize data due to format error, the declaration result shall be affected.
